Services must be used in the month they are earned or they are forfeited, members can transfer up to 2 complimentary services to a friend in a year if unable to use a monthly service 

(Transfer 1 service per 6-month interval).  

Consultation required for all services and must be deemed clinically appropriate.  Refunds will not be provided for rendered services.  6-month minimum membership required.   

$200 Early Cancellation Fee will apply if you cancel your membership before 6 months. 

Cancellations must be made by phone by calling 678-589-7546. 

After 6 months, you may cancel your membership free of charge.
