1. A copy of your driver’s license, passport, or other valid form of identification.

2. Your insurance card.

A. If you have lost your card, it is important to contact your insurance carrier prior to your visit in order to print a paper copy.

B. In the instance that we cannot verify your insurance and you do not have the card / print out handy, you will be held responsible for the total cost of any services at the time they are rendered.

3. For visits that require a referral from your primary care physician; you MUST have the referral (if your insurance determines it is needed), PRIOR to your appointment.

4. Please bring any other relevant items to your visit such as:

A. List of medications; over the counter or prescription(s), especially if extensive.
B. Any and all creams, or medications you are taking for the condition you are seeking treatment.

5. Accepted methods of payment include: cash or credit card (VISA, MasterCard, Discover, American Express). Personal checks are not accepted.
